Usually three or four sets of door shims, evenly spaced along the jamb, are plenty. Drive two finish nails into each set of shims to hold the jamb in place.
How far apart should shims be?
You would set shims such that when you level across those shims the tops of all the shims are level. You do want them fairly close together, so yes 12 to 16 inches apart.
